Mental Health Care Coordination
The ReHabilitation Center's Mental Health Care Coordination
program helps families whose child requires, or is at imminent risk of
psychiatric hospitalization or residential treatment facility placement, or
needs continued hospitalization. The
children served by this program meet the definition of serious emotional
disturbance (SED), are between the ages of 5 and 18 years, have complex mental
health needs, have service and support needs that cannot be met by just one
agency and/or system, and are capable of being cared for in their own home
and/or community if they are provided access to services. Participants in this program must have a
viable and consistent living environment with parents and/or guardians who are
able and willing to participate in the MHCC program and support the child in
the home and the community.
The MHCC program is dedicated to assisting children and
families in accessing comprehensive services and supports to ensure successful
living within the community, as well as providing a variety of supports to
their families. This program serves 24
children in three counties (Allegany, Cattaraugus, and Chautauqua).
